This test is a measure of serum cortisol (also known as hydrocortisone), an important hormone produced by a pair of endocrine glands called the adrenal glands. Its primary functions are to increase blood sugar through gluconeogenesis, suppress the immune system, and aid the metabolism of fat, protein, and carbohydrate. It also decreases bone formation. This test is performed on patients who may have malfunctioning adrenal glands. Blood and urine cortisol, together with the determination of adrenocorticotropic hormone (ACTH), are the three most important tests in the investigation of Cushing's syndrome (caused by an overproduction of cortisol) and Addison's disease (caused by the underproduction of cortisol). It also decreases bone formation. The level of cortisol in the blood normally rises and falls in a "diurnal variation" pattern. It peaks early in the morning, then declines throughout the day, reaching its lowest level about midnight. This pattern can change when a person works irregular shifts (such as the night shift) and sleeps at different times of the day, and it can become disrupted when a disease or condition either limits or stimulates cortisol production. In this test blood cortisol is checked at morning 8am.

DHEA-sulfate test measures the amount of DHEA-sulfate in the blood. DHEA-sulfate is a weak male hormone (androgen) produced by the adrenal gland in both men and women. The test for dehydroepiandrosterone sulfate (DHEAS) is ordered along with tests for testosterone and several other male hormones (androgens) to evaluate adrenal gland function, to diagnose tumors in the outer layer (cortex) of the adrenal gland (adrenocortical tumors) and adrenal cancers and congenital adrenal hyperplasia and adult-onset adrenal hyperplasia. In women, concentrations of DHEAS are often measured, along with other hormones such as FSH, LH, prolactin, estrogen, and testosterone, to help diagnose polycystic ovarian syndrome (PCOS) and to help rule out other causes of infertility, lack of menstrual period (amenorrhea), and presence of excess facial and body hair (hirsutism). DHEAS levels may be ordered with other hormones to investigate and diagnose the cause of the development of masculine physical characteristics (virilization) in young girls and early (precocious) puberty in young boys.

Estriol can be measured in maternal blood or urine and can be used as a marker of fetal health. Unconjugated estriol (uE3) is a form of estrogen that is produced by the fetus through metabolism. This process involves the liver, adrenals, and the placenta. Some of the unconjugated estriol crosses the placenta and can be measured in the mother's blood. Levels rise around the 8th week and continue to increase until shortly before delivery. The follicle-stimulating hormone (FSH) test measures the amount of FSH in the blood. FSH is a hormone that regulates the growth and development of eggs and sperm, and this test is used to diagnose or evaluate disorders involving the pituitary gland and reproductive system.
