This test is a measure of serum cortisol (also known as hydrocortisone), an important hormone produced by a pair of endocrine glands called the adrenal glands. Its primary functions are to increase blood sugar through gluconeogenesis, suppress the immune system, and aid the metabolism of fat, protein, and carbohydrate. It also decreases bone formation. This test is performed on patients who may have malfunctioning adrenal glands. Blood and urine cortisol, together with the determination of adrenocorticotropic hormone (ACTH), are the three most important tests in the investigation of Cushing's syndrome (caused by an overproduction of cortisol) and Addison's disease (caused by the underproduction of cortisol). It also decreases bone formation. The level of cortisol in the blood normally rises and falls in a "diurnal variation" pattern. It peaks early in the morning, then declines throughout the day, reaching its lowest level about midnight. This pattern can change when a person works irregular shifts (such as the night shift) and sleeps at different times of the day, and it can become disrupted when a disease or condition either limits or stimulates cortisol production. Since cortisol levels normally fluctuate throughout the day, a blood level is checked in the morning around 8 a.m. and again in the afternoon around 4 p.m.
